Use the form below to participate in the promotion
Please check the following participating vendors you are using in your circular. Participating vendors are subject to change. Please submit your selected vendor list to us by 2/15/2022. It is our expectation that your chosen vendors will include vendors that are on the approved FMG list. If not, you must have a prior agreement In writing from the respective vendor in order to be reimbursed for their product placement. Imagine will send a courtesy email - to notify each prospective vendor of your desired participation - based on this list. Should the vendor refuse to reimburse Imagine Advertising, a supplemental invoice may be submitted directly to you. Should you submit a revised vendor list after the deadline, you must include a written approval from the vendors you have selected.

    4-page tab/ 2-page broadsheet requires 4 pages or 8 - 1/2 pages. 8-page glued tab or 4-page standard requires 8 pages or 16 - 1/2 pages. Jumbo Postcard requires 7 - 1/2 vendors.
